[BUG] Claude code repeatedly presenting plan for approval

What's Wrong?
When using Claude Code on my iPhone in plan mode, the plan is repeatedly presented when I am either in a different session or in another app.
This has burned through a bunch of my usage credits without me realising as it appears the harness is recalling Claude each time on a ~5min basis which returns a preamble text and presents the plan.
I have attached screenshots of an example - each of these was unprompted by me and I got a notification for each one and burned ~$10 worth of usage credits without submitting a single query.
This only stops by opening the session and either accepting the plan for action or clicking provide edits and then clicking stop.
I have also experienced this when in plan mode and it asks me a question however this is far less common.

What Should Happen?
When told to create a plan the agent should build the plan, present it once and then wait for a user response.

Steps to Reproduce
- Create a new session in the iPhone app
- Request a plan be created for anything in your repo
- Leave the session (either by going to another session or leaving the app)
- Wait for the plan to be finished and you receive a notification
- Don’t open the session
- ~5mins later another notification is received saying the plan is ready for review and more usage credits have been used.
