[Bug] Persistent Hallucination and Fabrication in Code Generation

Bug Description
Claude started lying from the very first task; I was told by Anthropic that this was related to large ambitious code bases but it started lying on the modest step 1 of a greenfield project. 2 weeks of this mendacious behaviour. 2 weeks.
