[BUG] Ghost session consuming $130.46 in credits with $0.00 local usage

What's Wrong?
Environment
- Version: 2.1.215
- Platform: Windows
- Login: Claude Pro account
Bug Description
A ghost session is consuming all my usage credits server-side while my local
Claude Code session shows $0.00 cost and 0 tokens used.
What happens:
- Local Claude Code session: $0.00 cost, 0 tokens, 0 lines changed
- Server-side: $130.46 charged, session at 100%
- As soon as the 5h session resets, it goes back to 100% in under 2 minutes
with zero local activity
- Fable weekly usage shows 95% despite no Fable usage in weeks
- Stats tab shows a ghost session running for 9 days 3h 54 minutes
Steps to reproduce:
- Open Claude Code (no active project, no messages sent)
- Check /usage → $0.00 locally
- Check claude.ai settings → 100% session used

What Should Happen?
The session usage on claude.ai should reflect actual local usage.
Since my local Claude Code session shows $0.00 cost and 0 tokens used,
the server-side counter should not charge $130.46 or show 100% usage.
The Fable counter should also show 0% since I have not used Fable in weeks.

Steps to Reproduce
- Open Claude Code (no active project, no messages sent)
- Run /usage → shows $0.00 cost, 0 tokens, 0 lines changed
- Open claude.ai → Settings → Usage → session shows 100% used
- Wait for the 5h session to reset
- Within 2 minutes, session goes back to 100% with no local activity
- Stats tab in Claude Code shows a ghost session of 9 days 3h 54 minutes
- Fable weekly counter shows 95% despite no Fable usage in weeks
- Total charged server-side: $130.46 — all from promotional credits
