[Bug] Tool calls suppress preceding chat messages to user

Bug Description
Tool calls (e.g. question tool or memory tool) seem to be swallowing chat messages that the agent tries to print to the user. Several times, I've had claude tell me things like "The plan is above. read it and give your verdict" followed by the Question tool. But the plan never got printed. In other window, the agent thought it printed a plan review to me, but its memory tool call swallowed that print as well.
