[Bug] /output-style:new fails to generate interactive style configuration

Bug Description
I ran /output-style:new and it invoked a custom agent which churned away for a bit and then did nothing. the main agent said " Please share your preferences for any of the questions above, and I'll create a tailored output style configuration for you" but there were no questions. end of the day, basically a generic template was created in output-styles directory, there was no interactive component, useless invocation of a sub-agent. possibly due to running with --dangerously-skip-permissions(?)
