[Bug] Opus model producing incoherent responses and losing task context

Bug Description
I was extremely dissatisfied with this session, as the Opus model is performing well below the standard I am used to; the session demonstrated that the model generated code and responded to my requests in a confused manner, completely losing sight of what I had actually asked for. This is unacceptable, given that it is an expensive model that is failing to handle even simple tasks.
