[Bug] Claude misreads code and makes incorrect bug reports despite extended thinking enabled

Bug Description
Serious attitude. Really annoying, and then doesn't even read the files. It calls design choices mistakes, and when asking it for bugs in my code, it either doesn't read the file and provides bugs I already fixed, or calls out things that are genuinely not bugs, then backtracks about them, realizing they are solid code. It had thinking on so it should've thought through them first before jumping to conclusions.
