[Bug] AskUserQuestion tool response not displayed before follow-up confirmation prompt

Bug Description
I'm using Matt Pocock's Grill with docs, and Claude asked me a question using the AskUSerQuestion tool, where I can select 1,2,3, type something else, or ask to chat about it. And then in the end, it asked me whether "is this okay like this?" - but I didn't see the response it sent above it. So I had to deny the request and ask it to respond fully in chat. Not sure if it's a skill limitation or the model or the Claude Code temrinal UI. I'm using it in VSCode with a devcontainer and the Terminal view.
