[Bug] Internal command shadowed by plugin skill with same name

Bug Description
Internal /code-review — I could not launch it. The mattpocock plugin (https://github.com/mattpocock/skills) registers a skill named code-review, which shadows the built-in in my Skill tool, so bare code-review resolves to the two-axis plugin skill every time (security-review has no such collision, which is why it loaded the built-in fine). The internal correctness engine is a user-triggered slash command — please type /code-review high (local) or /code-review ultra (the billed cloud multi-agent pass) in your prompt and it'll run against this branch.
