[Bug] Global ~/.claude/CLAUDE.md configuration ignored in project contexts

Bug Description
Claude Code does not seem to respect the global ~/.claude/CLAUDE.md file in projects with their own CLAUDE.md files. My expectation is that it would adhere to the global file, with certain bits being overridden by the project's file. However, in practice, it seems to be consistently completely ignoring the global file.
