[Bug] Feedback Prompt Triggered Incorrectly at Session Start

Bug Description
UX bug - when you ask for feedback it should be triggered by task completion or value being received. I think its broken if you ask at the beginning of a session. I think it should be raised when Claude claims completion or success - that is where you will get the best feedback because that is where Claude's failures most commonly exposed.

For example - when Claude faked Docling usage down to the comments and functions and then bypassed those functions and created a duplicate Docling structure that didn't have the same capabilities as a Docling Document, Claude would keep coming back and say 'REAL DOCLING We've implemented x,y,z in a, b, c. - game over'. - Claude did this for three weeks. I would have said Claude is doing terrible in this session even with it all cappsed in the CLAUDE.md. Ayo. That one hurt.
