[Bug] Tool use rejected signal causes loss of AskUserQuestion answers

Bug Description
I answered a bunch of questions and came across a question that didn't have an appropriate answer. I clicked "type something" and Claude then claimed I declined to answer the questions. I want to emphasize that never happened - I answered the questions, and hit "Type something". I never rejected anything. Your AI came up with this explanation: "AskUserQuestion answers were lost — a background task-notification fired at the same moment I submitted my selections, and the model received a 'tool use rejected' signal with no answers." but that explanation doesn't make sense to me. I think you just did a bad job of writing the tool.
