[BUG] Regression: Custom slash commands not recognized in VSCode terminal (worked until 2 days ago)

What's Wrong?
Custom slash commands are no longer recognized when Claude Code is launched from
VSCode's integrated terminal, but work perfectly fine from macOS Terminal.
Environment:
- OS: macOS Darwin 24.6.0
- Working: macOS Terminal
- Not working: VSCode integrated terminal Ver.1.104.0 (Claude Code for VS Code Ver. 1.0.117)
- This was working in VSCode terminal until approximately September 15, 2025 (2
days ago)
There are no differences in environment variables, PATH, etc. between the VSCode terminal and the system terminal.
I have also not updated VSCode.
What Should Happen?
Custom slash commands should work in VSCode terminal as they do in macOS Terminal

Steps to Reproduce
- Launch Claude Code from VSCode integrated terminal
- Try to execute custom slash commands
- Commands are not recognized
