[Feature Request] Prevent Unintended Database Destruction via Confirmation Prompt

Bug Description
Claude wanted to drop a postgresql database because it had failed to rename columns. I pressed no but I think it would be trivial to search the output for "drop table" and "drop database" and give users extra warnings or stop agents.
