[Bug] Execution halts after shell command completion with context loss

Bug Description
這個和之前好幾個session （backgroung session）都會發生執行到一半在「 Ran 1 shell command」後就停掉。嚴重一點還遇過直接被退出 worktree 上下文也都被清空（類似 /clear 的效果）
