[BUG] File has been unexpectedly modified

What's Wrong?

when claude try to edit a file it fails with the error File has been unexpectedly modified. claude then proceed to re-read the file and retry, but the same error appear. this is CRUCIAL, because my claude subscription is useless now.

What Should Happen?

it should be able to edit the file
