[BUG] env var to control bash timeout not work

What's Wrong?

Bug verified in https://github.com/anthropics/claude-code/issues/5615. I tested the fix and found it doesn't work in the latest version.

What Should Happen?

Env var BASH_DEFAULT_TIMEOUT_MS set in bash should work.
