[Bug] Anthropic API Error: insufficient_scope not triggering step-up authorization flow

Bug Description
Claude Code should handle a 403 insufficient_scope response by initiating a step-up authorization flow per the MCP spec, but instead it swallows the response and surfaces a misleading "token expired" error
