[BUG] `tmux` pane name not reverted correctly.

Bug Description

Please can you correctly revert the name of the pane for tmux users.
The name of the pane remains the claude conversation name even after claude code is closed.

Steps to Reproduce

1. Run claude code in tmux.
2. Vibe code.
3. Close claude
4. Pane name is stuck to convo name :(

Expected Behavior

You should revert the pane name/description to what it was before.

Actual Behavior

(See img. ) Claude's session name remains the pane's name even after it's closed. Makes life confusing and makes me have to close the pane to reset the behaviour.
