[Bug] Claude ignores context files (CLAUDE.md) and prior implementations in conversation history

Bug Description
Claude continues to ignore memory files, CLAUDE.md, self checking agents and reinvent or ignore prior art. It is very difficult to align on any effort when a simple bug fix comes back as a complete redeign when the problem has already been solved, documnted, and previously implemented. Claude needs a reliable way to "remember" prior state, decisions, and design. It needs a self-check that can fire reliably. We are trying to do it now with agents, but the bottom line is without this claude is just a conventional coder with no critical thinking.
