[Bug] Composio MCP authentication loop on credential rejection

Bug Description
Tried authenticating Composio via Claude Code and it kept going in circles: ⚠ 1 MCP server needs authentication · run /mcp ❯ /mcp ⎿ Got new credentials, but composio rejected them on reconnect. Try re-authenticating, or restart Claude Code if it persists.
