[BUG] Tool rejections incorrectly triggered when user did not reject (Webstorm)

What's Wrong?

Description:

• Claude Code plugin incorrectly shows "User rejected update"

messages when attempting file edits

• User explicitly stated they did not reject the tool use
• This prevents normal file editing workflow from functioning
• Occurs when trying to use Edit tool on existing files

Steps to reproduce:

1. Attempt to edit a file using the Edit tool
2. Plugin shows rejection message even though user didn't reject
3. Tool use fails with "The user doesn't want to proceed with this

tool use"

This appears to be a plugin-side issue with tool rejection detection.

Also, on "Please restart your IDE or try installing" -- no matter how many times I install or restart this will not go away. Tried 3 times now.

the plugin should not report a rejection
