Feature Request: Show message timestamps in VS Code chat UI

Problem
The VS Code chat UI does not display timestamps. Session files have them but the UI hides them.
Proposed Solution
- Hover over message to see timestamp
- Optional inline timestamp display
- Date separators between sessions
