Claude in Chrome extension not connecting to Claude Code on Windows 11

Description
The "Claude in Chrome" extension is installed and I'm logged in with the
correct account, but Claude Code cannot connect to it. Every browser tool
call (tabs_context_mcp) returns "Browser extension is not connected."
Environment
- OS: Windows 11 Home (10.0.26200)
- Claude Code model: Opus 4.8 (1M context)
- Extension: installed from https://claude.ai/chrome, logged in as the same
account used in Claude Code
Steps to reproduce
- Install the Claude in Chrome extension, log in with the same account.
- Open the extension settings — shows logged in, microphone granted,
"No sites approved yet".
- In Claude Code, trigger any browser tool (tabs_context_mcp).
What I tried
- Fully closed and reopened Chrome.
- Fully closed and reopened Claude Code.
- Confirmed same account is logged in on both.
Expected
tabs_context_mcp returns the current tab group.
Actual
"Browser extension is not connected. Please ensure the Claude browser
extension is installed and running... you may need to restart Chrome."
The error persists after all restarts.
