[Bug] Claude makes unfounded claims without analyzing context or user constraints

Bug Description
if you look into this session history claude wants me to not drop the column from a table and dropping it will cause major granularity issue and duplicates..But I know for fact it wont..Its cooking up a lie without even doing any basic analysis or lookup. just simple plain claim without analysing anything. even though I particularly asked it not not cook up anything with its assumption
