What's Wrong?

症状: アシスタントのツール呼び出しが頻繁に壊れ、ツールが実行されず
"Your tool call was malformed and could not be parsed" となる、
あるいは何も実行されずユーザーから見て「止まる」。1セッション中ほぼ毎回発生。

環境: Claude Code (CLI) on WSL2 (Ubuntu-22.04) / Windows
モデル: Opus 4.8 (1M context) ※セッション途中でSonnet 4.5から切替

再現傾向:

• ツール呼び出しの直前に長めの説明テキストを置いたとき高頻度
• Bash/Write/Edit/AskUserQuestion など種類を問わず発生
• 「1メッセージ1ツール」にしても解消しなかった

影響: 同じツール呼び出しを何度も出し直すことになり、作業が分断され
非常にストレス。思考の連続性も失われる。
