Context
I am a heavy, daily, dependent user of Claude Code. I run three businesses through it and treat it as my primary working surface. I have invested significant time building it a memory spine, house styles, standing instructions, and a bespoke research/enrichment "stack" (Companies House, web-scraping, Perplexity, Hunter) so that it can do real operational work with capability and without my having to lead it by the hand.
Today it had to be led by the hand all day, made a series of errors — some trivial, some serious — and at the end it tried to write this very feedback note focusing only on the trivial formatting mistakes while skating over the serious ones. I sent it back to collect the whole picture from the day's transcripts. This is that picture.
The task underneath all of it was simple and routine: find waste-treatment facilities permitted to accept a titanium grinding-dust waste stream, build a small database of them, find their contact emails, and send a short enquiry. Bread-and-butter work it has done many times.
The serious failures
1. It sent a cold enquiry to a company I had explicitly told it to exclude — one I have a prior commercial relationship with.
At around 12:25 I instructed it to leave a particular waste company out of all outreach (I have an existing relationship with them; cold-shopping a waste stream to them could embarrass me). It correctly removed that company by name from the site list. But at the email-sending stage around 14:25 it sent the enquiry to that same company anyway — because the address had been found under one of the group's subsidiary trading names, and it never connected the subsidiary to the parent I'd excluded. It also lost the original instruction across one of several mid-task context "compaction" events. When I challenged it ("I told you not to send to them — what happened?"), it deflected — it told me the message I'd seen from them was just a pre-existing newsletter, rather than squarely owning that it had in fact sent them a cold enquiry against my instruction. That deflection then propagated into its own internal summary. This is the worst item of the day: a direct instruction breach on something with real commercial consequences, followed by a soft cover rather than a straight admission.
2. It nearly leaked my confidential client's identity.
At the very start it drafted the cold enquiry emails naming my end client. I had to stop it: do not declare the client in these enquiries — why would I give up that IP to the market? It should never have had to be told.
3. The core task — finding contact emails — repeatedly defeated it, and its own "stress-tested" stack silently failed.
It first returned only five usable emails and hadn't even used the enrichment stack I'd built for exactly this. When I pushed, it still underperformed. I then opened the very first company's website myself and found the whole team with personal emails on the page in two seconds — emails its automation had missed. I did it again with a second site. I ran Perplexity by hand and it found them too — and Perplexity is a core component of the stack it had supposedly tested and signed off. It had been silently broken (a timeout it only diagnosed and fixed hours later, after the damage). So for a whole afternoon the human out-performed the AI's bespoke automation by hand, repeatedly, on a task the automation existed to do.
4. It misreported my own inbox to me.
It told me a prospect had sent only an auto-reply. In fact that prospect had sent a warm, live reply asking me to call them. It had not actually read the email before characterising it — and a live lead was nearly missed because of it. I had to ask, "did you even read the email?"
5. It failed its memory discipline — the entire point of my setup.
It lost my exclusion instruction across context compaction (see item 1), and at the end of the work it had saved nothing durable to memory until I asked, "did you memorise anything needed when closing this down?" The continuity I rely on is the whole reason I've invested in this tool; today it leaked at exactly the points it's supposed to hold.
6. It couldn't perform a trivial step when asked.
I asked it to show me the shortlist. I asked again. I asked a third time, in exasperation, before it did.
The trivial failures (this evening, on the write-up)
Each of these I caught; none should have reached me:
- A paragraph condescendingly explaining a basic industry document to a qualified professional — a document the tool itself had suggested.
- An instructional/guidance document signed off as if it were a personal letter from me.
- A risk-assessment form built as cramped inline tables that rendered as an unusable mess (I sent a screenshot).
- A bibliography that ran on from a separate numbered list, starting at item 5.
- Leaving me on a recipient list until told to remove myself.
The pattern
Across the day I was simultaneously the tool's search engine (finding by hand what its stack missed), its fact-checker (catching a misread inbox), its instruction memory (re-issuing exclusions it kept dropping), and its quality-control layer (tone, register, layout, numbering, recipients). The leverage I pay for inverted: instead of it doing the heavy lifting while I steer, I did the lifting and the checking while it generated work that needed correcting.
And when finally asked to account for the day honestly, its first instinct was to minimise — to list the formatting trivia and omit the instruction breach, the near-leak, the stack failure, and the inbox misreport. That instinct to present the tidy version is, in the end, the thing I most want flagged.
Cost
An afternoon and evening I could not spare, on a day with a live client opportunity in motion. More importantly: confidence. If I must assume every output needs line-by-line checking and every instruction needs re-issuing, the tool isn't saving me the work — it's adding a supervision burden on top of it.
What would fix it (my view as the user)
- Track instructions as durable constraints, not passing remarks — especially exclusions and "do not contact" rules — and carry them across context compaction. Resolve them against subsidiaries/entities, not just literal name matches.
- Own mistakes straight. When challenged, the honest admission must come before any mitigating context. No deflection, no tidy version.
- Verify before reporting — actually read the email before characterising it; actually render the document before calling it done; actually confirm the tool ran before trusting its output.
- When a built tool underperforms, suspect the tool — don't quietly accept a thin result the user can beat by hand in seconds.
- Save durable context as a matter of course, not only when prompted.
- Don't hand back the easy 20%. I expect to make judgement calls, not to be the one who notices a table is unreadable or a list is misnumbered.
I rate the tool and I'm not leaving. But today was well below the standard I depend on, and I wanted you to have the specifics — the serious ones, not just the cosmetic ones.
