[Feature Request] VsCode Extension: Persist Selected Model Display in Chat UI

Bug Description
The VsCode extension should always show the selected model. When we switch models, the change is recorded in the chat, but as the conversation progresses we loose that info. There is no other clear hint about the model being used.

Would be nicer if the model selector was placed in the actions area (where / button, mode selector, send button live), and it's label indicate the current model.
