[Bug] Anthropic API: Excessive safety filtering on clinical terminology in medical domain contexts

Bug Description
Fable is repeatedly flagged doing sober software engineering on a menstrual-health app for endometriosis sufferers — the engineering vocabulary is inherently clinical and cannot be framed out. Three documented occurrences in one day, including a session that followed deliberate content-hygiene protocols. The same work proceeds without issue in long-context sessions.
