[Bug] Agent answers user questions instead of quizzing in interactive mode

Bug Description
The agent is supposed to be quizzing me, but keeps answering questions on my behalf.

Opus 4.8 / Claude Code 2.1.170, the assistant emits a usur/user turn marker and fabricates the user's reply plus fake system reminders within a single turn
