[Bug] SSH connections prompt for login and fail on keychain-based authentication

Bug Description
Claude Code no longer works over SSH from my iPhone - apparently tied to Claude Code design decision related to the macOS keychain that excludes SSH by default. Over SSH from iPhone it asks me to /login but that never works. Confirmed same login works locally on host but not remotely on the same host via SSH.
