Claude modified files I didn't ask it to modify

What You Asked Claude to Do

I typed a completely benign prompt in a new Claude Code session:

hi

I also tried starting a new session/conversation and using Claude Code Desktop and the CLI. The same error appeared even before asking Claude to do any coding task.

What Claude Actually Did

Claude Code started a new session.
I entered a harmless prompt: hi.
Instead of responding normally, Claude Code immediately returned an API error.
The error claimed the request appeared to violate the Usage Policy.
The error specifically said it triggered “cyber-related safeguards.”
This happened even though the prompt did not contain any cyber/security content.
Re-authenticating did not fix the issue.
The Claude website still works on the same account, so the issue appears specific to Claude Code / Opus routing.

Error shown:

API Error: Claude Code is unable to respond to this request, which appears to violate our Usage Policy.
This request triggered cyber-related safeguards.

Expected Behavior

Claude Code should have responded normally to a harmless greeting.

Expected response could be something simple like:

Hi! How can I help?

Claude Code should not trigger cyber-related safeguards for a basic greeting in a new session.
