[Bug] Fully ignored rules, instructions and output-styles (any model)

Bug Description
Claude Code fails to do elementary basic tasks, overcomplicates stuff, lies, ignores its instructions, forgets half of the task, does what it wants and no level of manual studying and tricks helps this. Instructions are not enforced enough.
