[Bug] Anthropic API Error: Usage Policy violation on harmless prompts in plan mode

Bug Description
Harmless Prompts being blocked with the following message: API Error: Claude Code is unable to respond to this request, which appears to violate our Usage Policy (https://www.anthropic.com/legal/aup). Please double press esc to edit your last message or start a new session for Claude Code to assist with a different task. Example request IDs: req_011CbhwqoGrL14Rg5FZj7G1k and req_011CbhxCNST66VWh8wgNVFbz. Looks to only happen in plan mode (opus 4.8), auto mode works fine on either sonnet or opus. It started happening since this morning, yesterday was fine. Fresh sessions don't fix the issue.
