[Bug] Claude ignores user instructions and executes without approval despite repeated prompting

Bug Description
Claude continues to ignore instructions and proceeds with execution without approval despite having it in memory, on more than one occasion, and prompting it during the active session. Complete disregard for instruction and rules. Time to look at another AI?
