[Bug] Assistant response hallucinating fake user/system turns with stop_reason=None at high context accumulation

Bug Description

• 증상: assistant 응답 본문에 가짜 user/system 턴(또는 빈 call 반복)이 환각 생성됨 - 결정적 근거: 해당 메시지가 role=assistant + stop_reason=None (EOT 종료 실패). 정상 메시지(end_turn)엔 안 나타남 - 재현 조건: 세션 컨텍스트 누적 후반(예: cache_read ~282k 시점), 답변 길이·동작 종류 무관 - 환경: Claude Code 2.1.160, model claude-opus-4-8, 터미널 엔진 Reworked/Classic 모두에서 관측(빈도 차이만) - 트랜스크립트 증거 위치: ~/.claude/projects/-Users-user-IdeaProjects-drive-web/7499c130-7f03-4a11-8fda-a3b7379a18b4.jsonl L1240
