[Bug] Serializer-complete rule violation in conversation state management

Question

**Bug Description**
Claude broke a rule twice in the same conversation, right after you picked "design surfaced first." No excuse for that — the serializer-complete rule is the single most
  emphasized thing in your CLAUDE.md and I plowed past it.
