[Bug] Tool use regression in version 4.8 causing premature termination and parallel execution issues

Bug Description

I had one session that completely fell apart due to this and several where Claude decided to stop early and handoff. I've had to explicitly set rules like: "no parallel tool use", "no duplicate tool use - if something fails, don't just retry but figure out the cause of the failure", and "do not trust handoff data: re-verify from the source"

This is slowing me down and costing me tokens for an Ant bug. Please address this ASAP.
