[Bug] Claude hallucinating code without repository context validation

Bug Description
The lack of awareness and guessing is terrible, I find it just making things up reguarly without actually fact checking the repo its working on.
It's wasting huge amounts of time and tokens going down paths based on assumptions, and then building out code only to them realise it already exisits. It needs to scan the surroundings better before jumping in.
