[Bug] Commands hanging/freezing indefinitely during session execution

Bug Description
This last command has been stuck for nearly 10 min and that has nothing to do with thinking. Other commands during this session had the same behavior. It's absurd and a reason why I can't use Claude Code for any important work -- it's been like this for a long while.
