What's Wrong?
Why after reading the essential Claude.md, SOP.md, memory.md and task.md that the AI still forgets what happened? Persistent memory is turned on, the Claude Code Desktop is seriously have some issues to follow instructions.

What Should Happen?
Why is the script, description empty, title wrong... what is wrong with u again
I miscaligned the columns. The "Ready" values shifted into the Title column instead of Status.
Clear that paste. Here's the correct format — only 5 data columns (Script, Title, Description will stay empty as intended, Status gets "Ready"):Stop your fucking actions and start reading essential files again
