[Bug] Ultrareview synthesis step crashes after backend completes findings analysis

Bug Description
Ultrareview crashed at "synthesizing" step on agentservice rc5 vs origin/release/sprint131/rc3-fix10 (82 files / +11172 / -25). Backend found and verified 8/8 findings, then synthesizer failed. CLI output: "Review failed: Review crashed before producing findings." Cloud session: https://claude.ai/code/session_01YYZVe1hTuZpEMJYLVXjaUt?from=cli Burned free run 1 of 3 despite no synthesized output. Requesting refund + investigation.
