[BUG] Max plan: 'Usage credits required for long context requests' on standard 400k context

What's Wrong?

▎ Bug: Claude Code returns Usage credits are required for long context requests on every prompt, even with standard 400k context (not 1M).

▎ Environment: v2.1.150, macOS, Max plan, Sonnet 4.6, curl install

▎ Context usage: 25.2k/400k tokens (confirmed via /context)

▎ Tried: update CLI, re-auth, /context standard, clear cache — none worked. Same account works fine on another machine.

▎ Debug log: API returns 429 rate_limit_error with message "Usage credits are required for long context requests."

▎ Request ID: req_011CbKbPEhPmpTGam5tRv7nu

▎ Expected: Standard context requests on Max plan should not require usage credits.
