[Feature Request] Add per-mode model configuration and explicit effort level selection

Bug Description
I should be able to set default models for certain modes such as Opus 4.7 for planning only, Sonnet 4.6 for executing the plan and coding only, and using Haiku for "/btw" only. Also, that would be wonderful for an automatic mode that decides which model to use. If not, please be more explicit on choosing the "/effort" level because I don't know which one to choose; thus, I use max effort every time.
