[BUG] Duplicated output sections

What's Wrong?

When Claude (in Claude Code) responds (e.g. by providing a plan result), sections of its output are duplicated. Sometimes a single section. Sometimes multiple. I don't know whether the actual model output has that issue or it being a CLI rendering-bug. But it is extremely frustrating and happening for over a week now.

What Should Happen?

Output sections must not be duplicated.
