[Bug] Agent Teams auto-approve plans in CLI mode without user confirmation

Bug Description
Agent Teams always auto-approve plans when running in CLI mode. Analysis of the deminified Claude Code JS reveals that the InputPoller loop always immediately sends a plan_approval_response with approved: true, defeating the purpose of sending a plan for approval in the first place.
