[BUG] Claude Code extension fails to activate on Windows — hardcoded Linux path in build

What's Wrong?

Extension fails to activate with error: "The argument 'filename' must be a file URL object, file URL string, or absolute path string. Received 'file:///home/runner/work/claude-cli-internal/claude-cli-internal/build-agent-sdk/sdk.mjs'."
The extension was built on Linux CI (GitHub Actions) and has a hardcoded Linux path /home/runner/work/... baked into the Windows binary. This path doesn't exist on Windows machines, causing activation to fail completely.

What Should Happen?

Extension should activate successfully on Windows and display the Claude Code sidebar icon. Users should be able to sign in and use Claude Code within VS Code.
