[MODEL] Plan mode indicator persists after ExitPlanMode is approved

Claude modified files I didn't ask it to modify

What You Asked Claude to Do

I asked for a plan and after the plan was delivered I asked for an adjustment

What Claude Actually Did

1. I was on "plan mode on"
2. After Claude produced the plan and showed it to me, it kept "plan mode on" but actually got ExitPlanMode approved under the hood (I didn't notice)
3. I asked for a plan change. Notice that "plan mode on" was still showing to me
4. Claude changed the plan and implemented it (because it had previsously had ExitPlanMode approved somehow)

Expected Behavior

The "plan mode on" should had disappeared when it got ExitPlanMode approved so I would know to turn it on again when asking Claude to adjust the plan
