[BUG] MCP salesforce not connecting set up as per document

What's Wrong?

I connected Claude to MCP earlier and worked fine for the whole organization recently this week or so something changes and the connection no longer works and it seems like when i try to reconnect it gives me error. please see error message below.

What Should Happen?

It should connect to our salesforce Production.

Steps to Reproduce

Here are the reproduction steps to add to your report:

Log into Claude.ai (web app)
Navigate to Settings → Connectors
Click "Add a connector" or "Connect custom MCP"
Enter the following URL:

https://api.salesforce.com/platform/mcp/v1/platform/sobject-all

Click Connect
Claude redirects to Salesforce login/authorization page
Log in with Salesforce credentials and click Allow/Authorize
Salesforce redirects back to Claude.ai
Claude immediately displays:

"Authorization with the MCP server failed. You can check your credentials and permissions. If this persists, share this reference with support: ofid_f416fb04f174281d"

Result: Connection fails. No tools available. New ofid_ code generated on every attempt.
Expected Result: Connection succeeds and sobject-all tools (9 tools) become available in Claude.

Reproducibility: 100% — fails on every attempt across multiple sessions
Attempts made: 4 separate reconnect attempts, each generating a unique ofid_ code:
