What's Wrong?

When renaming a chat in the Claude Code VS Code extension, the custom name reverts to the automatically generated name after the window is refreshed or reopened.

Open Claude Code in VS Code
Right-click on a chat in the history panel
Rename the chat to a custom name
Refresh or reopen the VS Code window
The chat name reverts to the original auto-generated name

The custom chat name should persist across sessions and window refreshes.

OS: Windows 11 Enterprise 10.0.26200
VS Code version: (add your version)
Claude Code extension version: (add your version)
